Our wellness programs are focused on assisting individuals in the aging process by providing a person-directed, proactive, holistic vision of health and wellness. Services are designed to promote healthy aging and stabilize chronic diseases.

    Our Rehabilitation Care services provide short-term nursing care and recovery support in a setting with the warmth of home.

    For seniors who wish to stay in their current homes, but find they could use some extra help to maintain their independence, we offer home health care and companion services in the Manhattan community.  

  • CenterPointe Physicians

    CenterPointe Physicians is accepting new patients of all ages!

  • Outpatient Therapy

    Meadowlark offers speech therapy, occupational therapy, physical therapy, and low vision services to seniors in the Manhattan community, as well as residents of Meadowlark Hills.
