Sending Patients to Bramlage House

Bramlage House is a transitional care facility that provides recovery and rehabilitation services. It’s located at the Meadowlark Hills campus in Manhattan. Our in-house staff provides continuing recuperation support, including physical therapy, occupational therapy, and speech therapy as needed. We also provide mental health services.

About the Facility

The 20-bed unit is staffed 24/7 with two registered nurses overseeing 10 beds each, plus a floor nurse who provides direct care and nursing aides. Each guest has a private room and receives highly individualized care, which have a positive effect on recovery.

The most unique thing about Bramlage is its welcoming atmosphere. It was designed with comfort in mind, so guests recovering there feel more at home.

Referral Process

Patients often stay at Bramlage to recover from surgery like knee replacement. Physicians can help patients pre-schedule a stay, or arrangements can be made during their hospital visit. Simply reach out to us and we can help facilitate the referral with your office, ensuring all the necessary paperwork and details are taken care of.

Bramlage is open to all eligible seniors needing recovery assistance, not just residents of the Meadowlark Hills retirement community.

When patients are discharged, Meadowlark will facilitate the transition in care by providing all medical records, medication lists, and doctor’s notes back to your office.

Guided Care

You, together with your patients, can determine who will guide their care while at Bramlage. Some physicians choose to make rounds at Bramlage personally. Others opt to have Dr. Matt Floersch, Medical Director at Meadowlark, follow patients while they are at Bramlage.

Care is largely nurse-driven, with a team providing individualized attention and support. Dr. Floersch makes rounds weekly, however, he is on call 24/7. The team can address problems quickly, which helps avoid rehospitalization or trips to the emergency room.

If needed, Meadowlark will communicate changes in status or concerns about patients to your office quickly so that you can continue to oversee care, even if you are unable to visit the facility personally.
